Abstract
UD. Anugrah is a medium-scale forklift rental service company that currently lacks a scheduled maintenance plan. As a result, the company frequently faces equipment damage, leading to downtime. UD. Anugrah has 15 forklift machines. The focus of this research is on the Komatsu 3-ton No.II forklift, which has recorded the highest frequency of issues 58 instances resulting in a total downtime of 251 hours. To mitigate losses, it is essential for the company to implement a structured maintenance schedule. This study aims to identify critical components of the Komatsu 3-ton forklift using a Pareto diagram and to establish the optimal preventive replacement intervals for critical components through the Age Replacement model. The results of this study identified 20 components that have experienced damage on the Komatsu 3-ton forklift and among these components, it can be found that the roller bearings are critical, with the highest frequency of damage, occurring 14 times during the period from 2017 to 2019. Using the Age Replacement model calculations, it was determined that the optimal time for preventive replacement is at the 53rd-day interval. The proposed cost for these replacements is Rp. 6,622,664 over a three-year period, requiring 15 replacements
