Abstract
PT. XYZ is a carton box manufacturer experiencing defect rate fluctuations in its production process. This research aims to analyze carton box product quality control using Six Sigma and RCA approaches to identify defect causes and formulate improvement strategies. The research methodology employs DMAIC stages integrated with RCA analysis. Production data from March-August 2025 shows an average DPMO value of 119,333.33 with sigma level 3. Pareto diagram identifies three dominant defect types: sheet warping 50.9%, sheet peeling 38.1%, and color mismatch 11%. RCA analysis reveals various causal factors from human, material, machine, method, and environmental aspects that contribute to defect occurrence. Improvement recommendations include material moisture monitoring, paper cleanliness inspection, and real-time roller temperature control to enhance production quality
