Abstract

The implementation of dicharge planning has not been done optimally. Discharge planning can prevent patient dependence and increase the independence of patients with diabetes mellitus. The purpose of this research is to analyze the correlation between the implementation of discharge planning with the level of independence of patients with diabetes mellitus at Anwar Medika General Hospital Sidoarjo. This research use analytic observational design with cross sectional approach. The number of population use was 33 respondents and 30 respondents. Sampling technique using purposive sampling method. Data collection using observational sheet of independence level of barthel index and checklist discharge planning. Data processing with frequency distribution table, cross tabulation and chi square statistical test. The result of research of discharge planning application of diabetes mellitus patient at General Hospital of Anwar Medika Sidoarjo is mostly not done (73,3%). The level of independence of patients with diabetes mellitus at General Hospital Anwar Medika Sidoarjo is a category that has heavy dependence (66.7%). The chi square statistic test shows the value of p <α (0,000 <0.05). Based on the result of research, it can be concluded that there is a correlation between the implementation of discharge planning with the independence level of diabetes mellitus patient at Anwar Medika Hospital in Sidoarjo. Patients are expected to perform and implement self-care according to what the nurse has explained in the discharge planning.
