Abstract

CV. Sumber Rejeki is a service company specializing in transporting goods with open trucks. The maintenance department is responsible for ensuring the fleet operates well, but accidents often occur during truck repairs. From June to November 2023, accidents happened every month during truck maintenance. This study aims to analyze root causes and determine solutions and improvement priorities using FMEA and RCA methods. The FMEA results indicate that hand injury accidents are the top priority for improvement. RCA analysis found the solution to be using chain-block tools for lifting heavy loads. In the future, it is necessary to perform regular maintenance on work equipment, monitor every work process, ensure the complete use of PPE, and socialize the SOPs for work procedures.
