Abstract
This study aims to implement an integrated application of two risk analysis methodologies—HIRARC (Hazard Identification, Risk Assessment, and Risk Control) and HAZOP (Hazard and Operability Study)—on the Sodium Tripolyphosphate (STPP) production line at PT XYZ. Within this framework, HIRARC is used to assess hazards and risks associated with each work activity (macro-level risks) through the parameters of Likelihood and Severity, while HAZOP examines process deviations and their root causes (micro-level risks) at key process nodes, including the mixing unit, rotary kiln, cooling unit, and storage area. The integration of HIRARC and HAZOP produces a more comprehensive risk profile compared to a single-method approach. Of the 12 work activities analyzed, four were classified as high risk, five as medium risk, and three as low risk. The HAZOP analysis identified nine critical deviations, such as operating temperatures exceeding set points, inadequate local ventilation, unauthorized personnel in forklift lanes, and indications of electrical short circuits. Practical recommendations include the implementation of a ventilation–agitator interlock system in the mixing unit, the installation of SIL2-rated temperature alarms in the rotary kiln, segregation of forklift and pedestrian pathways with access control, enhancement of permit-to-work and lockout–tagout (LOTO) programs, as well as improvements in housekeeping and visual safety signage. Overall, the integrated HIRARC–HAZOP approach provides a more holistic occupational health and safety risk mapping and enables more precise risk controls to reduce the probability and impact of incidents in STPP production
