Abstract

Occupational Safety and Health (OHS) is a crucial aspect in ensuring safe and sustainable production activities, particularly in fabrication industries with high accident risk. This study aims to analyze workplace hazards in the fabrication area of PT XYZ using the (HIRARC) method and the (HAZOP). A descriptive approach was applied through field observations, interviews, and analysis of accident records from June–August 2025. The findings show that the main hazards arise from welding, marking, cutting, and blasting–painting activities. HIRARC results indicate that all four accident categories fall into the high-risk level, with scores ranging from 18 to 27. The HAZOP analysis identifies deviations such as more sparks, more smoke, less ventilation, more friction, no PPE, and more chemical mist, which may lead to burns, respiratory problems, eye irritation, and mechanical injuries. Recommended controls include consistent PPE use, improved ventilation, optimized machine guarding, and strengthened SOP implementation. Overall, the study highlights that integrating HIRARC and HAZOP provides a more detailed and systematic understanding of hazard sources, making it a strong foundation for improving OHS management in PT XYZ
